PRE-EXISTING MEDICAL CONDITIONS
All participants with a pre-existing medical condition must complete and submit a Pre-Existing Medical Conditions Declaration form prior to the close of entries. These forms are to be submitted to the Luke Smith by email on email@example.com. However, participants are encouraged to submit these as early as possible.
Once submitted, the forms will be reviewed by SLSNZ and a decision will be made on whether a Competitor Waiver form is required to be completed.
COVID-19 Event Guidelines
Under the COVID Protection Framework, we will require everyone who is eligible (above the age of 12 years and 3 months) to present their My Vaccine Pass to be allowed into the event space. This includes all competitors, coaches, team managers, officials, event lifeguards, staff, parents etc.
A 'CVC Checkpoint' will open 1 hour before the event where everyone will come through, scan their pass and receive a wristband to wear for the full event.
